You have the legal right to access your health records!

Since April 5, 2021, a federal rule called the Cures Rule requires doctors and hospitals to give patients free, fast access to their electronic medical records.

What Is MyChart?

A secure app or website that connects you to your medical records. It is free to use and can help you manage your healthcare needs.

While MyChart is a commonly used app by healthcare providers, your hospital/doctor’s office may use a different app (PatientPortal, etc)

How can MyChart or other Patient Portals be used?

MyChart and other patient portals can help you:

  • See test results quickly
  • Message your doctor or nurse
  • Schedule or cancel appointments
  • Request refills
  • View your vaccination and visit history

How can I sign up for MyChart/another Patient Portal?

  • Get an activation code from your doctor or clinic
  • Go to MyChart website or download the app
  • Set up a username, password, and security info

Can you sign up without an activation code?

Most MyChart systems allow you to sign up without a code by verifying your identity through:

  • Name
  • Date of birth
  • Address or phone number
  • Last 4 digits of your Social Security Number (if applicable)
  • Matching records in their system

Example (Johns Hopkins):

  • Go to mychart.hopkinsmedicine.org
  • Click "Sign Up Now"
  • Choose "Sign Up Online Without Activation Code"
  • Fill in personal information to verify identity

What is the benefit of using a healthcare app?

It is an easy way to communicate directly with your health providers without having to call directly. You can also read notes from past appointments and keep track of upcoming appointments. Patient apps help you:

  • Save time
  • Keep you informed
  • Helps you manage your health without waiting on hold
